Community Health Worker II Job Requirements General Summary: This role is considered the minimum role for all Community Health Workers (CHW) within UMMS organizations who have greater than 12 months experience. Entry level CHWs who successfully meet CHW I requirements will advance to CHW II once they complete 12 months of experience. II. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified. 1. Leadership- works collaboratively on the team and assists team members with patient assignments when possible. A. Exhibits a positive attitude. B. Provides support and encouragement to the team. C. Interacts professionally with the team and providers D. Treats everyone respectfully and avoids gossip. E. Takes ownership of all actions and shows willingness to answer for the outcomes from those choices F. Works to ensure resolution of patient safety issues and conflict to create a safe and incidentfree environment 2. Program Practice/Care Delivery- open to change and proactive in the pursuit of ideas and their transformation into practice A. Competent in all aspects of basic care coordination services delivery: B. Accesses appropriate resources D. Follows program-based policies, documentation requirements E. Provides customer friendly approach to patient and family F. Uses teach back method for reinforcing patient education G. Can speak to/teach others emergency response processes 3. Program Practice/Care Delivery- Enhanced Focus Area A. While working under supervision, assists with program-based procedures, following established guidelines B. Carries out tasks according to the care plan and documents activity in Epic C. Completes the SDoH and related screenings, provides appropriate referrals and follow up according to patient specific needs D. After additional training and skills validation, perform program-specific skills under the direction of a nurse case manager e.g. blood pressure screenings, health fair presentations, etc. that are within the scope of practice E. Performs additional independent tasks that improves patient access to care: e.g. pre-visit calls, follow-up reminders, assists with application processes F. Contributes to the implementation of new initiatives relevant to the CHW role 4. Professional Development- responsible for maintaining competency required for the patient care setting. Focuses on advancing own development. A. Takes responsibility for attending educational offerings and in-services and advancing own learning B. Displays professional behavior in time and attendance, requests for time off, and management of leave accrual C. Reads email and other hospital communications timely and applies new information in daily practice D. Completes annual competencies on time E. Proactively monitors and maintains competencies and certifications with no lapses and without reminders 5. Professional Development Enhanced Focus Area A. Contributes to program-based education and learning activities B. Serves as preceptor, coach, mentor to new CHW staff and students C. Serves as a super-user for Epic EMR or department content skills expert communicating updates and sharing knowledge D. Supports implementation and team training on new workflows, or processes E. Serves as a department competency validator F. Actively participates in departmental meetings or on a shared governance council 6. Service Quality/Safety- models customer service behaviors demonstrating best practices for customer service and service recovery. Practices safely and consistently maintains a safe care environment. A. Takes an active role in ensuring compliance with regulatory requirements complies with service delivery, documentation and education requirement. B. Able to identify potential/actual patient/family issues and performs 1st level service recovery. C. Participates in some aspects of PI projects within the unit (data collection, audits, helping with posters, group projects, in-services). D. Promotes a safe environment of care acting on safety concerns; provides follow up to manager/supervisor 7. Service Quality/Safety Enhanced Focus Area A. Serves as a change agent in new workflow or other intervention that enhances patient safety and quality e.g. performing audits, reviewing and reporting on data B. Recognizes near misses and alerts appropriate staff C. Monitors quality metrics and reports on variances D. Participates in survey data collection 8. Program Operations- supports and contributes to effective clinic and patient flow A. Actively contributes as member of team (cooperates with peers, flexible with assignments, etc) B. Attends staff meetings and huddles C. Aware of and supports program governance structure D. Participates in program level decision making; provides feedback as appropriate E. Participates in department initiatives and surveys (Employee Engagement, Culture of Safety, Diversity, Patient Satisfaction, Learning Needs Assessment, etc.) F. Maintains standards for clinic operations (equipment maintenance, policies and procedures, handoffs among care team, etc.) G. Maintains employee and regulatory requirements 9. Program Operations- Enhanced Focus Area: A. Assesses and recommends improvements for effective programmatic flow and patient related outcomes B. Participates in team-building efforts in the program/department setting C. Fully participates in staff meetings and huddles D. Submits repair or helpdesk tickets for service and monitors status until ticket closed E. Contributes to peer review F. Monitors par levels, reports items requiring replenishing and stocks supplies 10. Assists with service delivery operations including but not limited to: A. Patient enrollment and discharge documentation B. Care Plan task updates C. Scheduling timely appointments D. Filing (electronic) E. Patient phone calls F. Retrieves and routes phone messages to the appropriate person promptly, ensures messages cleared from voicemail as applicable G. Contacts pharmacies, other provider offices, and the inter-professional team as necessary to meet patient care needs III. Education and Experience 1. High school diploma, Associated degree preferred 2. Minimum of 1 year of outreach experience in a health care environment, medical practice or community based setting such as social services, supportive services, personal care or related duties working with the adult or senior population, is preferred 3. BLS, required 4. Medical Assistant certification is preferred 5. Full participation in the day to day operations of the program; take a lead role in some aspect of day to day operations 6. Consistent demonstration of UMMS Values IV. Knowledge, Skills and Abilities 1. Basic computer skills, required. 2. Minimum rating of Successfully Meets on annual evaluation 3. Current on all mandatory classes/training 4. Proficiency in all skills required in their assigned area 5. Full participation in the day to day operations of the program; takes a lead role in some aspect of day to day operations 6. Consistent demonstration of UMMS Values.
